Duties Owed Form
This form is a declaration from the Property Manager to the Property Owner of duties owed in their relationship. It will be signed by the Property Manager and must be signed by the Property Owner as acknowledgement of receipt. The signed Duties Owed form will then be presented to the tenants to inform them of the Property Owners representation of the Property Owner. The Tenants will sign this form to acknowledge receipt.
Property Management Agreement Form
This form is required to begin the process of Property Management. Owners must completed the form, initial the bottom of each page and sign the last page. Every owner must initial and sign this form.
IRS W-9 Form
This form is required to be completed by all Prospective Tenants over the age of 18. It may be used to verify application.
Disclosure of Information / Lead Paint Form
Consent to Act
This form is required only in the event the Property Owner chooses to sell the property and allows the Broker to represent them and the potential buyer.

Residential Application to Rent Form
One application has space for two applicants. Use another application if more than two tenants over the age of 18 are going to occupy the property. There is a $40.00 fee to process each prospective tenant’s application.
Tenant Application Score Worksheet
Credit Authorization Form
This form is required to be completed by all Prospective Tenants over the age of 18. It will be sent to the credit bureaus requesting a credit report.
Residential Rental Lease Agreement Form
This form is required to begin the process of renting a property. Prospective Tenants must completed the form, initial the bottom of each page and sign the last page. Every person over the age of 18 must complete an application.
Pet Agreement Form
This form is required by prospective tenants who will have pets occupy the property. All uncaged pets must be included in the application. If approved, there will be a $250.00 pet deposit required for each pet.
Foreclosure Addendum Lease Agreement Form
This form provides the tenant with procedures in the event the property leased, is foreclosed upon.
